We would like to make your first visit to the practice as pleasurable as possible. Your first dental visit consist of comprehensive exam including oral cancer screening. We will take series of dental x-rays and provide a complete clinical diagnosis with treatment options best suited for you.
Please print the forms and bring them with you on your first appointment. If you plan to fill these forms on your visit, please plan to arrive 15 mins prior to your appointment.
